The Syracuse Community Health Center was founded on the belief that all individuals should receive quality health care services, especially those who might otherwise be excluded from the health care system. We offer comprehensive, direct and support services to families and individuals at 15 locations throughout Onondaga County.

Direct Medical Services

Adult Medicine

Our diverse staff of internal medical professionals provides care for all adults, including family medicine, and offers routine medical exams, sport, school and work physicals, adult immunization, and referrals to specialty services.

Pediatrics

Our dynamic Pediatric department focuses on education and prevention of childhood disease, and treats infants through adolescents for all health care needs. Well child visits, acute illness or injury visits as well as long term management of chronic illness. We offer a variety of disease management programs to assist families of all cultural and economic backgrounds.

OB/GYN

A full range of women's health care including routine GYN care, prenatal/postpartum care, family planning, fetal monitoring, Nurse midwifery and colposcopy.

Dental Care

Fully staffed dental department provides preventative care including cleaning topical fluoride treatment, sealant & periodontal screening, diagnostic & restorative services, emergency care, oral surgery and more.

Eye Care

Services include routine eye exams, eye glass prescriptions, diabetic eye care, visual field exams, cataract and glaucoma checks. (Salina Location only)

Podiatry

Foot care provided for health and comfort, with emphasis on issues related to diabetes or circulatory problems.

Walk-In Care (Urgent Care)

Walk-In Care provides for urgent health care needs without scheduling an appointment. Just walk-in when you're sick and see one of our providers. Medical and dental care is provided on site Monday through Friday 7:00 am to 11:00 pm, Saturdays 9:00am to 9:00 pm, and Sundays and holidays from 9:00 am to 5:00 pm. (Salina Location only)

Comprehensive Medicaid Case Management

Designed to assist pregnant and parenting women with children under the age of one, to assist with scheduling and attending regular prenatal appointment and well child visits. Assistance provided for developing healthy coping skills, linking to education or job training programs, securing supplies for babies, and obtaining day care funding and placement.

Individual and Family Services (IFS)

Provides individual and group counseling to persons and families with mental health issues, including depression, anxiety, eating disorders, parenting issues, sexual identity issues, teens at risk, and domestic violence. Counseling available for families, couples, men and women.

OAS

Treatment and counseling services for clients struggling with alcohol and drug abuse. A state licensed program. (Salina Location only)

HIV Services

Counseling, testing education, primary care and referral services available for HIV/AIDS related treatment and issues.

Ancillary Services

Nutrition Counseling

Education and counseling for problems associated with diabetes, high cholesterol, high blood pressure, heart disease and obesity. Preventive care provided to help clients achieve a balanced diet and healthy eating habits.

Pharmacy

On-site pharmacy at Salina Site allows for same day or call ahead filling of prescriptions. Same day drop off and pick up available. Open Monday-Saturday during regular health center hours.

Radiology

Radiology department works with all medical departments to provide diagnostic x-rays, mammography, ultrasound and bone density testing. Extended hours available for walk-in (urgent) care.

Laboratory

On-site facility at Salina Site provides routine lab testing during regular health center hours.

Support Services

Childcare

Childcare services are offered to health center clients at no charge at appointments during regular business hours.

Translation

Translation services are available at all SCHC locations. Check with each location for hours or to request a translator during an appointment.

School Based Health Center Services

Syracuse Community Health Center and The Syracuse City School District have partnered to provide convenient access to health care for children. School- Based Health Centers are available at six District Schools - Dr. King Elementary, Dr. Weeks Elementary, Delaware Elementary and Bellevue Academy, Fowler High School and HW Smith Elementary.

Each School-Based Health Center operates during and beyond regular school hours, all year round, including all holiday and summer vacations. Participating children must have a completed form on file, obtained at each School-Based Health Center. School-Based Health Center services are provided whether or not your child has health care insurance coverage.
